All Client Profile sections and items require accurate and full completion unless otherwise noted. Leaving any part blank will delay processing. The completion of Client Profiles must precede and be submitted with all applications proprietary deferred and immediate variable annuities and fixed annuities as well as non-proprietary deferred and immediate variable annuities, fixed annuities, equity indexed products, variable life and group annuity / 401k products offered through AXA Network. Client Profiles are recommended but not required for compliance with the firm's Fact Finder policy for proprietary variable life and brokerage products. If at a later date the customer's profile significantly changes, the FP should update the Client Profile and appropriate records and review investment allocations and strategies with the client.
